We keep up with the digital age by utilizing an advanced electronic medical record (EMR) system allowing us to better serve our patients, while reducing our impact on the environment.
If you are a new patient, you may wish to have your medical records sent to us from your previous provider’s office. If you have their information available, you can fill out this form – and we will retrieve those records on your behalf.
If you are moving out of state, or wish to transfer your care elsewhere, you can fill out this form – and we will forward your ECMA medical records to your new provider.
