First Visit

On your first visit you will be asked to present proof of medical insurance, like an insurance card, and you may be asked to clarify information previously filled out on line. You will be asked to review our HIPAA compliance form and initial it, as well as our financial agreement form. We may also be asking you to sign a release of information for your previous healthcare provider, should you desire your records to be transferred to our facility. We will be collecting your co-payment at this time as well. You will have only a brief wait before being seen by your provider.
